Try the Mazzoni Center in Philadelphia. If they can't help you, they can at least give you a reference to somebody who can. Their website says, "Transgender services include: primary care, hormone therapy and monitoring, supportive counseling and referrals as needed. While we do not provide hormones on site, we have researched various methods for patients to obtain affordable hormonal therapies that meet their special needs."

My friend who's MTF is going through their process right now; she says they're really nice and helpful.
